Volunteer tutor sign-up

I give permission for Ozarks Literacy Council to conduct a Criminal background check by means of a personal identifier search (name-based) which searches information based on the individual’s name and date of birth. The fee for this background check is paid through the generosity of Ozarks Literacy Council donors.

By clicking the submit button below I give permission for Ozarks Literacy Council to use the information on my registration form for placement purposes and for statistical and grant/financial applications. I understand that my name and address will not be released to other people, businesses, or agencies without my written permission.
